Do not apply adsense until you have certain volume of traffic. This is extremely important. When advertising do not put ads on landing pages. Never tell anybody about you are making money from adsense.

Only if you are not big enough. For example deviantart has soft porn content yet they display adsense ads .)

Setup a business and use a business account and tax id to register for your Google Adsense account. This is very important because once banned, its based on your SSN or EIN. Anyone can have multiple accounts on Google Adsense as long as they are valid EINs they will be treated as separate entities.
You will not be told in most cases why you were banned from Adsense, expect that it will happen and then you will not be suprised when it does.
