Check your Search Console data. If I see a high impression count but a tanking click-through rate, the intent definitely shifted. Also, look at the SERP changes for your primary keyword. If the top results are now all video or listicles instead of long-form articles, your content is outdated.

For me, when the page starts loosing rankings and traffic but nothing has been changed on the site. That's usually when I check if users are looking for something different than what the page currently offers.

what i do look for is a steady decline in CTR and rankings with impressions remaining relatively stable. This is usually an indicator that Google is prioritizing a different type of content for the query.
 
The organic rankings without AI overviews give us direct evidence of user intent mismatch. When the ranking drops from first position or top-3 positions without overall drop in traffic, the page is clearly not matching user intent. It usually happens because people visit our website, then go back to visit some other webpage. They are not happy with out webpage - it could be content, loading speed, site structure, or some other reason. We should check the content of competitors and test by improving our content.
 
For me first change is always title tag and meta description, before touching content body. If SERP shifted toward different format like more listicles or videos, sometime just rewriting title to match new format brings CTR back without full rewrite.
If that doesnt help after couple weeks then I move into restructuring content itself. Starting small save time since its sometime purely click through issue, not actual content relevance problem.

one such element that I have observed is that of query drifts. this is where the page begins receiving impresions for relevant keyword and not the main keyword thereby making it appear differently in the eyes of google since there is no intent match.
 
I think many people focus only on rankings, but intent is more important. A page can still rank while no longer answering what users actually want, which eventually hurts traffic and conversions.
