Have been building quality PBN links to my site, targeting several money KWs.
Now this is happening as I build them:
Money KWs - Rankings stay the same or drop a little
Overall website organic traffic - Rises steadily and substantially

Disavowing domains certainly sounds like a plan. However, I haven't been too clean with my linkbuilding (eg. using PBNs etc).

1) Yes. Keyword planner is your best friend.
2) Hope I understood you correctly but some packages include on-site optimization (eg. alt and heading tags). It is safe to say that the link building services are purely off site.
