I've wondered this too, but then I see the same news story syndicated on multiple sites, and they have no penalty. Of course, Google propaganda says that they do penalize duplicate content. It is not clear though. My counter-example above, says otherwise.

Mate, I've lost 5k before, not with AdSense but a different ad provider. One that is quite popular too. They claimed fake clicks but I ran 100% whitehat campaign. Be glad they cut your losses at $88 and not further down the line when you have invested much more What "flags" did you set off?

If they are really good at making bots, probably not much you can do. But most people don't know how to hide it well enough. Check user-agents, IPs, screen size, fingerprint the entire browser and look for patterns and mouse movements

I've heard many horror stories with PayPal. Sometimes they take your money and there is nothing you can do, especially if it is a blackhat account.
