I sometimes think that Matt cutts and those chosen ones who know "How the search engine works" or what factors are needed to rank..can easily make TONS of money by ranking for such terms than what they get paid by Google..So surely some of their team members might be exploiting their power and authority due to which we see the BS sites listed in top 10..I am sure its not that difficult to them keep a check for such terms since they hire genius people from around the world.

This is pretty standard stuff in competitive niches.

In simple terms all that is happening here is-

1) create a page on an authority site (through illegal means in this example)
2) Insert hidden links into authority sites (again usually hacked with c99madshell or rogue plugins)

An EMD site ranks in the viagra serps with a network of sites all hosted on the same IP address and registered to the same person as the EMD.

Having links from related sites seems to override everything else in some cases.
