I would be calling today to follow up on the email answer any questions he may have and hook him. In my past just sending a email even though the customer asked for it will sit and sit and may never even be opened.

1. Call them back and see what they're wanting -- you can't offer them something until you know what they want
2. Find out what their budget is
3. Find out what their expectations are
4. Tell them that you'll write up a proposal for them based upon what they want and email it to them by the end of the week.
5. Now start looking for DEPENDABLE people you can outsource to and find out what their prices are.
6. Piecemeal the project. You never want one person to know who your client is and what hand the project over to them.
7. As soon as you know the fees to charge for the services your dentist wants THEN email them a proposal
8. Tell them the fee and how you expect to be paid and when
9. In the proposal, tell them if they are agreed, to email you back stating they agreed.
10. Follow up with their office manager the next day to see if they received your email.
They will either contact you or they won't. It's now in their ball park.

here you go, find out what he want's you can pretty much get anyone on fiverr to make a webpage and if you need some seo you can get that too. And just charge a butload of money not to mention maintenance and you are just outsourcing the work.

How do you take a SEO project in steps so that the person that has been hired to outsource the work doesn't take over the project? You said "You never want one person to know who your client is" and that's very true but whatever you do, you will need to give the domain name etc. to this person in order for them to do their service hence they will know who the client actually is, especially when you come up with multiple clients at a time. How do you handle that? Because you can't just keep changing the person that is doing the work as this will cause problems in finding someone that is good at what they're doing?

There are 2 ways to deal with this :
1) Ask the client to get WHOIS Privacy. Considering the client is spending thousands a year on SEO .. he can surely put $20 into this.
2) This is the technique used by most of the expert SEO :
What you do here is .. instead of getting backlinks to the client website directly .. you first setup 4-5 blogs of your own with the same targeted keywords. You do the SEO for your blogs. Once you have a very good PR .. you 301 redirect to the clients website.
All the PR juice goes to the client .. and then you just do some basic backlink building for the client website and bookmarking etc etc
This is also the reason why .. websites with just 50 backlinks are able to rank #1 for very competitive keywords ..outranking websites with thousands of backlinks.
Another advantage of this technique is .. if the client stops paying later ..you just remove the redirect and the client sees a nice fall in the traffic and comes running back to you

if not self hosted, the client will see from their stats that theyre receiving referrer traffic from wordpress/blogspot etc and by definition, will see that they can emulate your performance (or at least thats what they'll THINK they can do), but then again, answering my own question here - it isnt possible to do a 301 on something you dont own - right? right.

Yes, you have to self-host these blogs.. no wordpress, blogger blogs .. your own .coms and .nets.
If you really want to do the way it should be done ..then you need to create a nice link wheel. A small example would be :
a) 5 self-hosted blogs - linking to money site
b ) 15 free blogs ( blogger etc ) - linking to self-hosted blogs ( 3 per 1 self-hosted blog)
c) Social bookmarking, link building ( to the free blogs ), RSS feed submissions etc - All for just the free blogs
d) Last step - Build links to the bookmarks and profile links, scrapebox links etc
You will get a rough estimate of the setup now. ofcourse you cannot redirect blogger blogs etc.
