The cats of instagram page https://www.instagram.com/cats_of_instagram/ over 10 m followers and around 0.86 engagement rate as per phlanx...how do they make money?

I don't think they make much from shoutouts because -
1. I haven't seen any shoutout in this page for a long time.
2. People who are into IG would definitely check engagement rate before buying shoutouts and I dont think anyone would buy shoutout at 0.86 engagement rate.

Their website is just for uploading pics and videos.

So, they just do it for fun?

Some pages are just for clout, people do it to leverage it into other money making opportunities.
Can't say about this particular page though. But if I had a 10M page, I know various ways I could leverage it without ever doing a shoutout on the page.

0.86% engagement for 10M is never bad thing. Also there is more than 12K post in their page so it automatically reduces the engagement rates.
I have followed the page since long and they make money from various things like organising events, from partnering with various organizations or maybe via licensing videos. People would be eager to pay thousand of dollars if they just features their video on Instagram.
They don't spam their valuable page with dropshipping ads because it's one the biggest animal page in instagram and have verified badge.
So Lots of ways to make money for them.

Maybe they do it just to have something excellent in their portfolio.

Imagine that you meet a bigger prospect, trying to close a deal on managing/growing their corporate social media for $XXX(X) per month.
They ask you what you have done before, what successes you achieved in the field.

It's always nice to have/manage a 10 million follower account in such cases

No, Phlanx engagement calculator is crap. It averages ONLY the last 10 posts, which is absolutely worthless. You must give a post 24-48 hours if not more to get to it's full reach.

Averaging only the last 10 posts when a page posts 5-7 times per day is a worthless stat, considering 40-50% of those posts are less than 24 hours old.

They need to calculate based on last 30 posts, or something more accurate than 10 LOL.

By this logic @TheRock is also crap because he has 0.78 ER. All big pages have around 1,2 ER witch is normal for big pages.The bigger the page the lower the ER.

each person has different ways to monetize.
Maybe it's not their only page and they build properties that they will be able to sell in the future for x3-x4.
Ask them about their plans - maybe they will help you

Imagine running a 10m followers page and bumping in a product like a fancy comfortable cat bed selling at 80 dollars.
You're the affiliate, so you would only get around 5 to 10 dollars.
Now assuming the 0, 001% of your followers buys it, you bank in $xx.xxx
