there are multiple ways you could do this depending on how technically skilled you are. kids used to do the same type of shit with steam avatars. the only problem comes if they limit how often you can upload a new profile picture

I know this thread is old, but the way he does it is quite simple - his profile image is a picture hosted elsewhere. The picture is generated by a script on a timely manner. Thus, when you watch his account, the image always reflects the current time, as Instagram loads it from the remote server where it is stored. Simple!
