would say 99% of ad networks will accept any neich not just crypto sites only ones that are a little harder to find ad sites for is adult gambling and pharmaceutical sites

I am still waiting for activation of the Adsense account, Media.net after a few bigger withdrawals has blocked the account, and the other like Ad Maven, Revcontent networks do not accept my site.

Is there any network with less requirements/similar to Adsense&Media.net, which offers at least $0.3-0.5 per click for US insurance traffic?

Infolinks? Is it meaningful to try with PropellerAds / RevenueHits?

You can try Content.ad or AdBlade, but if you were blocked you'd better find out what was the reason so you won't face denial again.
The niche of your traffic has very little to do with CPC, as ads may depend on user previous browsing history and interests.

Direct means Publisher > Advertiser.
If that's Publisher > Ad Network > Advertiser it's not a direct ad.
