[WTH] Need Web Developer - Need a form built ASAP

I need someone to build me an oder form. I have the designs and schema all done.. you just need to translate this into real life for me.

So this is how it will work:

2. evaluate how much you will charge
3. PM me with your quote (make sure your actually specify the full price) and Skype name.
