The 5 fields data are the same for this 1000 forms, just key in the same thing, i will give you the value to key in.

What kind of Captcha is it? Are the pages identical?

If yes, I can do the job. I can easily make a macro or php script to do it automatically.

Update:

I had made an iMacro for it, all you need do is click on the yellow page listing, then run my macro by press "play" and then fill up the captcha and press submit.

Not all the yellow page listing have email, you only need do those with email. Those with personal email instead of send email then you need key yourself, let me know your price for 1500 entries (only maybe about half or 1/3 need fill up with my iMacro) .
