[WTB] Simple wordpress website with Unsubscribe form

Hey i need someone who could make me a simple wordpress website with unsubscribe form.

What is need is this: Add a link in html like this: <a href="https://yourwebsite.com/unsub_form">Unsubscribe</a> and the url should point to a form where people can input their email address that they want to unsub.
