How do i setup different Meta Titles & Descriptions for every page.
By default it just picks up the title & some random content from within the blog posts.

How do i manually change Meta Titles & Descriptions for every page/post ? Maybe some good plugins?

You can use "All in one SEO pack" for this. By this you can set meta tags and descriptions for every page.

Yeap, all above mentioned plugins are good, I tried them all. Seo ultimate was using a little bit more resources so I switched to wordpress seo plugin.

a bit of custom php code is your friend in this case
you can , with one carefully coded bit of php in your head.php file, create a unique dynamic meta description for every page on wordpress without having to statically set it up, and with minimal resource usage, and if you do it right, you can even get it to have proper keyword content for matching meta descript/title tag/highest density keyword
