WordPress website - need advice for a form

So... I have a client who he needs a WordPress site. I can do everything that he wants but cannot find a good way for a form.

What I need to do:

On the homepage there will be a button. It's going to redirect to that form. It may be on another site or hosted on the clien's one. What the form should contain:

1. Select 3 images
Poll with images
2. Attach a file
3. Write in the box
4. Select pricing plan
5. Pay 50% of the plan amount


Is there a possibility to make that using WordPress? It may be a paid plugin, that's not a problem. It may be another site that supports this scenario.

You're going to most likely need to have this functionality custom developed. I'm assuming you can't do this yourself? If that's the case, hop onto freelancer or upwork and pay someone $50 to do it for you. Can usually have it completed and ready to go within 24 hours.
