Wordpress theme w/ Contact form on home page?

Hi guys, I am looking for a wordpress theme w/ a large contact box/form on the home page... similar to this http://templatic.com/demos/?theme=appointment
Does anyone have any suggestions? I have been searching forever and cant find a good one.

check out gravity forms which is on the forum here. see if it works for you. Should work with almost any WP theme

I'll toss out another option you may want to consider. Pick ANY theme you like and then add a contact form plugin to a PAGE.

Then set the page as your home page on your site instead of the blog.

Also the one you showed as an example looks like the contact form is just in the sidebar. I believe you can add a sidebar contact form widget to a theme and set it to display only on the home page.

Just some ideas in case you can't find exactly what you are looking form
