I am seeking for a plugin that allows user to fill in fields like Name/Gender/Birthday/Phone number....


I need it to be fancy so that it can catch the users' eyes.

Any idea ?

Plugins are not good for that, you need lead capture services. As for the look, this is the easiest part - can be done with CSS.

You could use Contact-7 Form plugin (with custom CSS) and it's additional settings to save the information to the db.
