Hello,
Basically we want to send an email to all of our customers requesting a review with google/yell/facebook if they want to leave a positive review, but send them to a contact screen if they want to leave a negative review..

Is there a theme/plugin which can be used can do whats required?

This is called a 'review funnel'. The simplest way is to create a page with 5 star images. Link the stars to whichever page you want. If they click 5 stars they're redirected to a certain site.

There are many other considerations such as the referrer. There are a lot of providers who have review funnels already set up, if this is your first time you might want to see what they're doing already.

I have trialed grade.us which works fine, but i cant justify paying their monthly subscription, when i don't need any of the analytics or other features, I was hoping i would be able to find a standalone theme which would allow me to create the page.

Raise your service quality and prices until you can justify the cost of a review funnel tool.

I agree, I just checked and grade.us is $90/month. If you don't have the budget for that, you need to increase your rates and offerings.

I'm not a service provider, I'm trying to help the company i work for generate more customer reviews.
we have a fantastic reputation in our local area, but we don't have any online reviews, so i was looking for something i could email to our customers.

I would have thought i could get a developer to create a 'funnel' for $100-$200, so within a couple months i would be saving money, so that's the route ill probably be taking.

Just FYI, I've done this before and Yelp was able to filter based on the referrer address. You'll have to blank the referrer but better is probably to give it a fake referrer.

So they can stop exactly what you're doing, soliciting reviews. They're very aggressive for what they think is spam.
