Wordpress: Need Custom Feedback Page/Form, with BackEnd Collection

Title says it all! I just need a landing page that has a custom feedback collection form, with a backend collection that stores the feedback in order. It would have to have a password login, and settings for me to change the feedback form. Dont know what is the best way to do that? If its best to make that into a blank theme, or a plugin? And How I would access the backend, I guess through wordpress? or Static PHP?

Just need a custom collection form, with multiple features, and a backend that stores the data. So I think you will need to know SQL maybe? Maybe also PHP? Im not sure....
