I use Pretty Links for all my wordpress affiliate links, its really easy to set up and the lite version is enough for most peoples needs.
I setup groups in there relating to each page / product group to keep all the links organised and add each affiiliate link seperately, making sure i check the nofollow box (for affiliate links) and shorten it to the product keyword.
So a pretty link would be - http: //example.com/keyword
I make sure i add the keyword into the alt and title tags depending on whether its an image as well for consistancy.
Make sure you backup the database and site when you've built the links. When i first built a site using this method i installed another plugin later on that crashed the site so had to recreate nearly 50 links in that plugin. It was painful but thats what happens during the learning curve. I now automate all the backups so i dont worry about that.
Another bonus is the plugin tracks the clicks the links get in the dashboard so you can replace poor performing products with ones that get a better click thro rate.
This is working well for my affiliate sites, and the code in my theme is clean

Same here. I use pretty link. It's easy to use and works well!
Another thing to mention, Im sure you have to setup the permalinks to /%postname% for the method to work as i mentioned. This is better for seo purposes anyway but every page then had the same look and feel as your affiliate links. When someone hovers over the link it looks like just like another page on your site.
