Wordpress Contact Form Plugin With Attachments

Hi guys,
I'm looking for a WordPress contact form plugin with the following capabilities:

1- Can attach files (doc, docx, pdf, jpeg, txt, etc)
2- Can attach video files especially from mobile devices (smart phones and tablets).

Paid or free plugin, does not matter as long as it can do the above.

Contact form 7 is free and can attach files. Never tried from mobile tho.

You can create a contact form with the capabilities you mentioned, using CaptainForm. It's a WordPress form builder plugin. You can also create other types of forms like registration forms, feedback forms, lead generation forms, surveys, polls and many more.
