Word Press Form Tracking in GA?

Hi All,

I have word press form integrated on the various webpages of the website. I want to track the form fill outs in GA as conversions. How can I do that?
The Thank you page is a third party website URL (an app webpage).
1. How can I track the conversions?
2. Can I also track the details of the person who fills that form?

Please help me out with this. Thanks in Advance.
