WooCommerce Help! Product add on forms

Im hoping somebody can help. I am working on an E-commerce site using WooCommerce! I have purchased a plugin that lets me add extra options and forms to a product page.

However, When I add a new option it is not adding the value to the total price. I am getting a jquery error in the console.

I have added a screenshot. Any help will be much appreciated!

Thanks man, although Im still having no luck. Changing the codes is not working. I've contacted support but they have yet to get back to me!!
