It can happen. It can be linked based on the device and IP you are using. It can also be linked if you are using the same phonenumber on multiple accounts.
I wouldn't be to worried if you are doing normal stuff.
If doing automation or blackhat, if you value your other account don't risk it.

Usually yes. If you're not an advance programmer and you don't know how to have multiple accounts on the same phone without them being linked then they will be linked

Is there any way to have a fresh restart on an iPhone without creating a new Apple ID? For example when you delete the app, it still keeps the data. Is it possible to make it so it's like the first time downloading the app?

If you are going to use only two accounts , it is not a big issue.
