Why would this happen?

Yeah, this is happening sometimes. Meta can treat the same ad differently according to your account or page history. You can also check the rejection reason first.

yes this can happen and even with the same ad or the account history and trust can affect the review so just check the rejection reason and account quality first.
 
It can happen even when everything looks the same. The platform may handle each account differently based on its history or automated checks. I’d check the reason given for the declined ad, since that’s usually the best clue.
 
It happens alot and most especially when it involves BH ads, you are also lucky that they didn't completely ban the two account when they rejected one of the ads acc.

This is quite normal for experienced advertisers; we need to understand that Facebook currently has a significant margin of error. You simply need to create a new campaign, and it will be approved.

This issue happen on meta and other platforms as well. Ad reviews are not only based on the ad itself, but also on the account quality and history. Since you mentioned these are two old accounts, the previous ad history might also be a factor.
Also, AI review systems are not perfect and sometimes they can make mistakes or flag things incorrectly, you may try making some adjustments and testing again to see if the result improves
 
Check the account history first. One account may have more old rejections or restrictions, that can explain the different result.
 
There are various other differences between the two accounts, including varying trust levels, and different IP addresses or domain names may also cause one ad to be rejected while the other is approved.
