I think it largely comes down to your content; the remaining issues can mostly be easily fixed right when creating the campaign.
I’ve run into this before and sometimes the reason isn’t obvious from the rejection message I’d first check the ad copy landing page targeting and account history one by one Usually there’s a specific policy or setup issue causing the repeated rejections

I’d say policy violations are usually the main reason since even small issues can trigger rejection.

From my experience the content policy issues are usually the main reason so i would check the ad content first.

most of the time its the ad account or policy flags formatting is rarely the main issue

If you have campaigns rejected its because the Meta's review system is catching something thats violating the guidelines
Correct the ad creatives and the text. if u cant do that then use cloaking techniques from the ads dashboard itself
