Whixh wordpress form plugin can do this

I need a wordpress form plugin that can get "current page" url and then add it to the form entries.

you need to use some PHP code or javascript to do I think it's easy to make something like you want if you know a little bit og code

unlikely to find a plugin do something so specifically tiny
-look at your usual contact form plugin documentation. it may be in there.
-make multiple contact forms, one for each page you want, and in the template have it include the active page
-it's simple objective and good reason to learn a bit of php/code. answer is likely on stackoverflow for you to implement with copy/paste code job.
