The best way to find out is to open the inspect element inside the form area and you should be able to figure out the plugin name from CSS classes.
If you don't get the name, simply look for the common CSS classes from the code on Google.
Plugin detectors use the same methodology, in fact, sometimes they are not even correct. Nothing beats manual inspection of the source of the site.
