I guess the mods know (and are trained to know) that to err is human (is natural for human beings to make mistakes). And when the mods see some pattern indicating that this is not a mistake, then the mods often do crack the whip.

This can happen sometimes. If repeated intentionally multiple times, then this can be a problem.

Accidents happen, and I'm pretty sure I've done this before as well.

Sometimes, you just skim a thread and quickly compliment the content. Then a few months later, you come across it again, read it properly, and comment again.
