Whats a good way to implement a custom form in Wordpress???

I need to have a way for clients to sign in to a profile and be able to check a form that has multiple fields that I need to update regularly. These would be some of the fields:

I think you will not find such plugin, because you need to make custom work in any case. You can create such form with many plugins (gravity form, ninja form, formidable). But you need also to make solution for user profiles, for connection form with dynamic data, etc
