I shouldn't think your computer IP address being blacklisted would have any effect on your websites ranking unless maybe you host your website locally. Are you using any tools on your home computer? Without using proxies?

I'd get rid of that link if I was you. It contains your email address and IP address.

I'm not sure what the problem would be but I doubt it's associated with the drop in your rankings. I'd say they were two different issues. I'd also run a virus scan on your PC just in case it's being used for illegitimate purposes.

The ranking will go down even you did nothing. The ip ban won't affect your ranking anymore.

Either you or someone else on the network attempted to spam and the IP got blocked. It happened me once when I forgot to enable the proxies, but the IP got removed once I followed the instructions on their site (Stopforumspam).

And if your site went down in rankings then you must have used Scrapebox or something else...
