What software to create this kind of 3D map?

Lots of good options for that. Blender is probably the best as it's open source and well documented.
 
The photo is definitely shopped.

For the model (and for accurate contouring) you could definitely use GIS software (Q-Gis, Arcmap+Arcscene) which are pro-grade surveying/mapping tools.

Then again if you can live with creating the contours and the 3D part of the map you could use Sketchup, Blender, and 3DSMax.

Start of with grabbing the outline of the island from Google Images or from a GIS resource, then load that in any of those tools I mentioned, and work your way up from there :)
 
I'm almost certain the only model in there is the actual island with a diffuse map (texture) and a bump map to make it look like it has bit more depth. Any basic 3D modelling program is able to deal with that stuff, you would just have to scuplt your geometry and then apply the maps to the mesh. Maya - Blender - 3DSMax will all work for this. As for the rest looks like the model was rendered with no background and imported into photoshop along with the post-production stuff.
 
You can use Sketchup for getting more better results than this one if you are professional and mature in this field. It'll provide you 3D results easily with charming effects.
 
ok lets make it simple
1.open blender
2.create a simple plane object
3.And if you have the map apply the texture to it
4.And now also convert the image of the map to diffuse map and bump map you can google it
5.And apply them also to the plane and here is your 3d map and it you have the image of the map without the white background you can use this method
it will work for this thing also

And about the marked points in the top of the island i think they are added afterword and are 2d i think so you can do it using photoshop:):):):):);););););););););)

There is lot of 3d softwere 3ds max maya blender c4d all of theme could make this kind of map.

I second sketch-up, especially if you don't want/have time to invest learning blender. About 2 hours max is all you need to get some decent results out of Sketchup
