what is the structure behind SAPE?

ok, ok, I know "it's a place to buy/rent links"

but my question is "what is the structure of SAPE?" is it a bunch of sites volunteering to have a link on them?
is it hacked sites? the owners of the sites have no idea the links are there?
How is it different from BMR? if it's just a blog network, why can't they deindex them like they did to BMR?

If you need particular extensions (as .es or .fr), you will find a lot of hacked websites, but for .ru or .com there are a lot of site owners that need money.
