Thats a question, can't answered easy. So if your Website atm have a lot of bugs, needs hours to load etc, than you should make a redesign and clean up the code and the content. If your site is valid and loads very fast there isn't a must to change the design.

I mean only visually.
Let's say I have an established authority website with lots of good content.
Then I decide to fully change it's design. Won't it affect my SERPs?
I heard sometimes this happens. And people with good ranks hesitate to change something in their visual design because of possible drop in their positions.
So, the final answer is YES or NO?
In my opinion it's not true, but I want to be sure.

As an SEO expert making $20,000 per month I KNOW that design changes does NOT affect your Google rankings, however content changes definitely affect them. So as long as it's only your design being changed and not your content you should be fine.
