Webpage design needed

I am in need of a pretty basic website. NJeeed someone who can take what I have already got and just add some stuff to it. I need 3-4 more pages that are just informational and then a page that has a form that users fill out and submit. the form will need to have 5 file upload slots. Shoot me a PM or reply and I will get with you. Need this done ASAP.

I have a quick question.On which platform you developed it? is it wordpress or pure Html,css? I can do it within your time limit and budget if it is developed on wordpress.
