Very simple video intro needed, tamplate OK.

How's it going guys. I am looking to see if someone can help me out with a small intro to a video I am creating for a project. Our group had to create a project for our chemistry class and I am editing the entire video. Since I am not too experienced I don't really know how to create the intro. I am using a trial version of adobe premiere pro to edit the rest of the video. I see these video intros done on Fiver all the time, but I am kinda in a hurry to get this done in the next day or two.

I am hoping someone has a little extra time, I can send you $5.

How long does it need to be, and how much content do you have ready to be entered?

About 10-15 seconds, and its only going to have a title and maybe a subtitle. that's it.
