If I were to use English sim cards but use proxies that are in America, would this cause a flag with Instagram? I imagine it would.

It's easy and cheaper to get the proxies for American locations and easy and cheaper to get the Sim cards for England (where I live).

Does anyone have any information on using numbers from different countries?

    Thanks

I've only used different locations (proxy/sim) and don't run into much trouble

Nope, you won't have a problem doing that, been using UK sim cards for quite a while now with US proxies/accounts in FB. IG/Twitter/Google+ same thing as well.
