Think you need to check before copying and pasting info as that is quite old. Twitter verification is on old and there is no 'form' anymore. The verify accounts then,selves. There are some that go through another route (asking for verification due to copyright/name infringement) but that is hard to get unless you are well known and someone is impersonating you.
