Transfer Software - Web Submissions > Telephone Calls

We are seeking a live transfer solution to handle inbound calls in the UK,

We have clients filling in online forms - once they insert their telephone number (and submit) they should auto-dialed and then transferred to our sales floor.

Is there a software application to do this?
