Do you get penalized for having to many affiliated links?
For example, is it bad to hyperlink title, image and 'more info' button to the same affiliated site?

In my experience it's not a good idea to have a ton of affiliate links in a post. No more than 1 per 100-150 words. People have suffered types of penalties for too many.
"image and 'more info' button to the same affiliated site" I had an affiliate review site with this type of layout, I could not get it to rank. I changed the theme and toned down the quantity of links and it started to get better pretty fast.
As for hyperlinking title, I have never tried such a thing nor would I.

It would be ok if you are using url shortners but a lot of affiliates networks get marked as spam on offer links so try making your own.

Cloak them links out for bots and spiders. Detect real users and then show the links to them.

Too many affiliate links can result in a penalty, but 3 isn't "too many" IMO, so it's ok.

URL shorteners cannot hide your affiliate links from Google as Google will follow the link and discover that it redirects to an affiliate link. If you want to hide your affiliate links, you need to cloak them.

You will be fine with one at the beginning of the post and one at the end. You don't want your site to look spammy.
