Too many clicks, no conversions

Hello guys, attached you will find the statistics of my recent web traffic campaign targeting CAN
Niche: Chargeback/ Recovery

Landing page is cloaked, and is overall quite appealing and responsive but i dont understand why im not getting any form submissions, i got only 2 leads in total, could it be that ive not setup pixel?

Except that i wanted to ask the experts here what are some key tweaks on landing pages to maximize conversion rate.
