title and description auto changed

i am using wordpress with yoast free version ,i am facing a issue that title and part of description is automatically change on some of my pages instead of home page and some time keywords meta is empty while i am not seeing any error or warning alert for yoast or on my filemanager

Do you use any plugins except yoast? For this might be the reason. Problems with compatibility.

i have 4 plugins for right now

The list seems to be legit. Did you try to switch to the default theme? And how does this automatic change happen?

i did but problem exist and this not happens sudden but after to refresh for some time a page it happens
