Tier 1 / Tier 2 ISP networks

Here again with another inquiry.

Global market's are interesting. What's difficult to imagine in 1 country may be common in another. That being said, please allow me to ask what may be considered a naive question.

How and when do tier 1 ISP's provide IP's? I'm aware the nature of my question might seem delusional but honestly, stranger things have happened.
Let's assume you're a medium sized ESP, with financial ability.. how would a company gain access to favorable routes? Of course I assume reputation counts. I've noticed ISP's in old Eastern Bloc countries are a bit easier to purchase services from.
