You are advertising outside your geo or your business dosnt have enough traction online

1 thing is that if it is a Search ad, then please make sure your website, keywords, ad headlines and ad descriptions are all in sync with eachother. Kindly do not ask me to elaborate on this, as it is pretty much self explanatory.
