Tax ID Question

My question is, can I use an EIN instead? I know it doesn't say "EIN," but I'm kind of assuming they're just asking for a Tax ID.

Self-Employment income. You need to do a 1040 SE for taxes and report income/loss on Schedule C of the regular 1040.
