OP - fill out the forms, bank hard, keep track of your expenses, deduct your expenses and pay your share so you don't end up paying fines or worse yet, spending time in a place where you can't go home at night.
I'm in no way a tax professional and not even living in the US (assuming you are from the good ol' US of A?), so keep that in mind when contemplating my advice.
The reason for example Amazon needs you to fill tax forms to receive affiliate income is that they need to report it to the IRS, which would make it not that far fetched to assume that CPA networks need to report to the IRS as well.

Problems might start arising if you start earning good money and they'd possibly require an ID verification or perhaps you'd need to have a PayPal account under that same identity etc.
Personally I'd fill it out with my own information, just as long as the network is trustworthy and not going to sell said information to some Nigerian prince looking for a quick buck.

I'll keep those things in mind and im pretty sure most of the networks won't share the info.
However, if i can avoid filling out a tax form, i'll do it as it more money in my pocket lol. So do you guys know any "trusted" networks that don't require people to fill one out or do they all require one to be filled out?
Any trusted networks are going to comply with the law and require you to fill out the proper documentation.
