Has anyone knows how configurations should look like after I want to track form submission only from the specific page? I already set up form submission generally for all of the pages. I tried to use "Fire this trigger when an Event occurs and all of these conditions are true" but it doesn't help me...

Adjust your existing form submission trigger. Add a condition for 'Page URL' where you specify the exact URL of the page you want to track. Choose 'equals' or 'contains' for the match type, then input your specific page URL.
