T-Mobile Subsidy Codes - HELP

I only used the phone with a prepaid plan I purchased on eBay. I tried contacting T-Mobile, but they told me the name on the account doesn't match. This is crazy! I don't know why it's different. And here's the issue: I need this phone unlocked by later this afternoon since I'm going out of the country.

Can ANYONE help me get the subsidy code for this phone?
