squeeze pages

couple of questions

does a squeeze page need name and email address? whats peoples preference. also i want to design or have designed a squeeze page, to link from my ppc to collect opt-in for aweber. diet niche

would anybody kindly give me a link to their squeeze page so i can see what is expected?? i`m not going to copy but want to educate myself on layouts used etc

For all niches, at least 2 fields: name and email address.

If you are intended to promote information products or affiliate products, the 2 fields are enough.

e.g: If you promote a local service and need to meet people, you need 3 fields: name, email address, phone number.
