I know it might sound controversial, but there are two different POV comes to my mind.
  1. The first one is "survival of the fittest," or an even more basic form that implies if I can get something, then that thing shall be mine. If you run your life on such a simple form, you might win in some cases, but your loss is inevitable. Given your condition, under normal circumstances, your wife would do the same or the woman you're sleeping with will sleep with someone as close as your brother.

  2. Now, the second POV. Your actions shall not cause harm (mentally or physically) to any party.
    If you cannot handle it properly, you can make it easier by adding one or two clauses to the end of the previous sentence. For example, by adding "whom I care for" to the ending of the statement, you're simplifying the idea in a way that should better suit your needs.

Finally, you should choose the first or the second one. You can choose somewhere in between and find your thresholds as well. Personally, I have chosen a middle ground between the previous ideas with cheating as my redline. I have had many partners in the past couple of years, and I've told them on our first or second date that cheating is not acceptable and will not be tolerated.

I believe while you're still in that honeymoon-like phase, you won't come off as rude, and the other person will not take it personally.
 
Remember 1 thing ... KARMA ... But, KARMA sometimes takes time to execute itself, but it surely does and when it does, it is minimum 10x in return.

From what i understood is the whole guilt / fear is what if my wife does the same to me in future if i get some disease or i get fat or if i go unattractive what if the lady who brings joy and makes this house happy will do the same if i get into the situation like that guy ! i think this is what keeps bothering me , hope i never end up like him ,because i love her so much and i wont let it happen ,that's why i feel sad for him , may be he was also like me when he was healthy keeping the family happy and protects it !

If you're truly dissatisfied with the potential outcome, I'd suggest stopping your affair; however, keep in mind, such a thing is just a potential outcome and not an absolute occurrence.

There are many people who got away from doing way worst, but we never know how they'd ended up, as they are too smart to get caught. With that said, you have an option to quit early, minimizing the risk, or tag along and see what will happen.

I have told her multiple times to stop it ,she told me if you are willing to do it ,I will find someone else ,trust me i know her for many years she is a very nice lady she has a teenage son ,they were in madly love after he diagnosed with some medical issues which she doesn't want to discuss he started losing / couldn't perform well in bed she wanting it more plus I was satisfying her all these times i don't think if i leave her she will be able to manage on her on ,sure she will find someone.
