I have a site that has been linked with bad shitty pages. They only link to my images from my website.
Does a linked image affect me more than a anchor text link or is there any differences?

It depends, there is never a right answer when it comes to negative SEO.

However, you can always use the disavow tool from the GSC.

My bet is that noone even tries doing negative SEO to you.
These are just doorways hotlinking your images for their pages.
There's a big probability that 99% of that spam will never get indexed by Google. But to sleep better at night you can disavow those linking URL's/domains.

Do those images contain alt tags filled with your target keywords?

Alt tag is the anchor text for image.

If yes, you might run into some over-optimization issues.

What if your site has been hit with 1000+ of spam links? How do you disavow that?

The best tool for disavlow is Google Webmaster Tools.

download your back link profile to see who is linking to you also get data from some tools like semrush, aherf and mejestic..there might be some difference in accuracy and reliability of each tool. so get detail from all the tools and cross referencing and remove duplicate. once your list of bad back links ready you can submit to google console for disavlow.

Are those spam links showing in GWT? Basically im asking are they even indexed?

How old is your site? How big is it?

Use the disavow tool to block those shitty domains. But I believe the anchor text link will effect more than linked image.
